Your affiliate request cannot be processed until we have received your W9. If you already have a digital copy of your W9 please email it to support@debbieshadid.com. If still need to fill one out please click here and after filling in please choose the option to have it emailed to our support inbox.

Your Paypal Email address is where payment will be sent after every quarter.
